Transitioning to Adulthood: Empowering Young People to Thrive

Helping Young People with Autism, Learning Disabilities, Mental Health Challenges and Complex Behaviours Transition into Adulthood

We provide person-centred supported living services that empower young people to transition into adulthood with confidence and independence.

Supporting young people as they step into adulthood

Specialist supported living services tailored to autism, learning disabilities, mental health challenges and complex behaviours.

Key Challenges During Transition

The journey from childhood to adulthood can feel overwhelming for young people with complex needs and their families. Common challenges include:

A "Cliff Edge" in Support

Transitioning from children's to adult services can feel like stepping off a cliff, with gaps in care or delays, leaving individuals and families uncertain about the future.

Lack of Early Planning

Insufficient early planning is one of the most significant barriers to a successful transition. Without clear goals or a structured timeline, young people and their families can feel overwhelmed, and they may miss out on essential opportunities for growth.

Limited Opportunities for Meaningful Engagement

Many young people with learning disabilities and autism face limited opportunities for meaningful engagement, which can lead to isolation and a lack of purpose.

Communication Barriers

Communication barriers can prevent young people with learning disabilities, autism, mental health challenges and complex behaviours from fully expressing their needs, goals, and preferences during the transition to adulthood.

How Transforming Support Can Help

At Transforming Support, we offer specialist supported living services designed to bridge these gaps. Our approach focuses on:

Early, Coordinated Planning

How We Make a Difference: We ensure this never happens by providing seamless, well-coordinated transitions that prioritise continuity and stability. From the moment a young person is referred to us, we work closely with families, carers, and professionals to create a robust transition plan. This includes overlapping periods where young people can become familiar with their new supported living environment while still receiving support from their existing services. By fostering collaboration among all stakeholders, we ensure a smooth and supported transition into adulthood without disruption to care or well-being.

Person-Centred Collaborative Approach

How we make a difference: We prioritise early and structured planning to ensure every transition is purposeful and positive. Our process begins with a comprehensive assessment of the young person’s needs, aspirations, and potential challenges. This allows us to identify key milestones and create a personalised timeline that gradually builds their independence. By involving the young person, families, carers, advocates and professionals early on, we ensure alignment and a shared vision for the future. Early planning also helps us address potential service gaps, guaranteeing consistent support throughout the transition.

Meaningful Engagement Opportunities

How We Make A Difference: Transforming Support focuses on creating opportunities that empower individuals to lead fulfilling, active lives filled with purpose and growth. Through our supported living services, we help young people explore their interests, discover new hobbies, and engage in activities that foster personal development. From education and volunteering to employment and social groups, we ensure access to opportunities that are both meaningful and enjoyable. By encouraging participation in their community, we help individuals build confidence, form connections, and develop a strong sense of belonging.

Effective Communication

How We Make A Difference: We take a proactive and inclusive approach to ensure every voice is heard and understood.

Our team is trained in a variety of communication techniques, including Makaton, visual aids, and assistive technologies, to support individuals who may find verbal communication challenging. We also work closely with the people we support, families, advocates and professionals to identify the most effective methods for each young person.

By fostering an environment where individuals feel safe and confident to express themselves, we tailor our support to their unique needs and aspirations.

Outcomes For People

T's Story

T's arrival at our Horley service presented significant challenges, as he was the first person we supported in the service with notable historic behavioural issues transitioning from children's services to supported living. Initially, he showed little cooperation, with frequent shouting, throwing food at the manager, and unauthorised departures to the seaside that required police intervention and multiple missing person reports. This was also a challenge to the family, which felt unsupported by the system. The dedicated staff at Horley implemented a strategy of gradual engagement. They focused on building trust through shared activities, offering personal care support, and assisting with room cleaning. This consistent, patient approach has led to a remarkable transformation.

T's Story

T now regularly informs staff when he leaves and returns, and he reliably responds to text messages when out. He has also embraced community life, participating in daily lounge activities like card games and forming friendly relationships with the manager and other residents. His journey is a testament to the power of a supportive environment and tailored care in fostering trust and promoting positive change.
