Transitioning to Supported Living
We understand that moving home is a significant change, no matter what you are moving from and where you are moving to! For the people we support, we ensure that the move to supported living is carried out as smoothly as possible.
Some individuals have spent a long time in a hospital setting, so getting used to the freedom of their own homes is a challenge. We understand that no matter how exciting this move is, it can also be scary and anxiety-provoking. We want to support people in the right way so that their move is successful, and so just as ongoing care and support are personalised, is the moving-in process.
For some individuals, moving in is done in a phased way, perhaps starting with a weekly visit for an afternoon, building to a daily visit and then sleeping over one night per week. This can then be increased to staying longer each week, at a comfortable and appropriate rate for the individual, until they are ready to move in completely. This can be done over months, and there is no set time for the client to move in.
